I proclaim that i haven't perceive any symptoms on my child: fever / sore throat / vomiting / diarrhea / skin rash / icterus / other serious skin lesions, skin suppuration / exuding eye disease / pustulous ear and nose flow. Child is lice and scabies free. I proclaim that the submitted personal and medical details are assigned and contribute to preservate it until 5 year. Contribute every photo or video that taken about me can be usable for the popularization of the association and surf sport. The proclamation is filled by Ministry of National Resources decree. The child's legal representative 4 days before the camp, confirms the child medical status. The necessary documents for the proclamation are given by the camp organizer, to fill out. Must return the filled documents until the stated date. On camp starting day at the latest, have to pass the proclamation to the camp organizer. The proclamation must be kept below the site of camp during the camp. I declare that my child can swim and accept if he/she drinks alcohol (under 18) or use drugs, or any which harms, Our Association will obligate to leave the camp, notify the parents and parents will manage their child travel to home. Our Association won't pay back the camp fee in this situation.
